I have some promising news.  I've been going through and
building/testing some older Ubuntu kernels.  I think I found the last
version that does not exhibit the bug:  3.13.0-16.36

I've been testing it for 12 hours without hitting the bug.  I'll test it
for a little while longer to be sure it's good.  If it is, I'll start a
kernel bisect and identify the offending commit.  I see a couple that
stick out in the git log, but more testing is needed.

I'll post additional updates shortly.
