Did this issue start happening after an update/upgrade? Was there a
prior kernel version where you were not having this particular problem?

but had the same issue. Tried 15.06, still ahd same issue, then tried
16.04, still same issue.
I tested '4.6.0-040600rc4-generic' and I am still experiencing the same
issue.
