Public bug reported:
While attempting to recover "/" partition after an operator error, reinstallion
of Ubuntu 14.04.1 Trusty Tahr failed shortly after beginning the installion
process.
Partitions were selected, formatted as needed, and installation began.
Shortly after setting username and password options, the installer crashed,
with the attached bug report.
