I have seen what appears to be the same problem in both 15.10 and 16.04
distributions: windows that sometimes end up away from the visible
workspace when using a dual-monitor setup.
I can recover them with 1) alt-space and maximize, or 2) disconnecting
the second display.
I have a Lenovo Thinkpad X230, with Intel graphics.
As #12 suggests, I am now trying to disable the session management
plugin through CCSM, and see whether I'll see again the problem.
