Public bug reported:
Remove support for the "old-style header" from AdaptivePageLayout (APL).
Currently there is the (internal) subHeader to take care of this, but it
is not well tested and we cannot support it with all features in the
future.
This will force apps that use APL to make use of PageHeader.
