Public bug reported:
1. Upgraded from 15.10 to 16.04
2. Click MySQL Workbench - has "slow fade" in and out for a few seconds.
MySQL WorkBench fails to launch.
3. Launch Terminal. Type mysql-workbench and enter to launch - this is
successful.
4. Now there are two icons in the Unity launcher bar.

5. Unlock old icon from launcher and lock new icon to launcher (if it didn't
already lock automatically)
I think this is a bug and whatever has changed in
/usr/share/applications/mysql-workbench.desktop should be updated during
upgrade...
