This may be a bug in Pulseaudio introduced in version 8 - at least it
was in my case. The issue is fixed in the newest release of Pulseaudio
(Automatic routing improvements) -

*It did not appear in Ubuntu 14.04 (which uses version 4) and does appear in 
16.10 (which uses version 8).
*I could fix it through brute force be making the *-default-sink file in 
~/.config/pulse ro.
*I fixed it through the temporary fix as outlined in this post - 
https://lists.freedesktop.org/archives/pulseaudio-bugs/2016-February/010460.html.
 Please note that this may introduce other regressions. Read the entire post to 
find out what. The regressions should not affect the vast majority of users.
