It looks like these three patches are only needed in Trusty, since they
were included in mainline as of 4.1 and 4.2:

I built a trusty test kernel which can be downloaded from:
http://kernel.ubuntu.com/~jsalisbury/lp1447485/

Can you test this kernel and see if it resolves this bug.  If it does, I
will submit a Trusty SRU request.
