A couple of style suggestions: "Closes: LP: #XYZ" sounds odd to me,
because Closes is usually for a Debian bug.  Convention is just to use
"LP: #XYZ" or "(LP: #XYZ)". But your style does work.

Also, I'd refer to the bug number once and mention the details for each
patch in sub-bullets. That saves any confusion with multiple mentions of
the same bug (which isn't obvious unless you look carefully).
