Issue:
When attempting to hot-insert a virtual disk into a virtualized Guest that is
online, the Guest OS encounters an error.
On the host, the Guest has been created and started, then hot-insert a virtual
disk to the Guest:
# virsh attach-disk Guest1_Xenial --source /mnt/a/data_disk.qcow2 --target vda
Disk attached successfully
On the Guest OS, check for available disks but the newly attached disk was not
found, then check /var/log/kern.log, we see this following error:

Although the newly attached virtual disk was not recognized/seen by the
Guest OS right away, upon rebooting the Guest, The newly attached
virtual disk will show up and become available. However, expectation was
that the virtual disk will be immediately available in the Guest OS upon
attaching it and without having to reboot the Guest.
Experimentation has been done by slightly modifying the attach-disk
command, wherein the target was changed from vda to sdc (sdc because sda
& sdb have already been defined for Guest). Although the Guest no longer
sees the aforementioned error in kern.log, the newly attached virtual
disk still does not show up in the Guest until the Guest has been
rebooted.
