Public bug reported:

Hi :)
Actually it's a Multiboot Usb booting a ".iso" image, which i torrented down to
~/Downloads and then cp'd across on the Usb stick /media/sdb3/boot/grub/iso

I'm still trying to figure out how to get persistence working and
haven't tried updates and upgrades yet.

I'm trying to install Ubuntu 16.04.1 onto some old partitions that have very
old and broken versions of Ubuntu on them. With past versions of Ubuntu i've
been able to avoid formatting during install and that;
1.  fixed any problems
2.  refreshed my system
3. kept my old stuff even without needing to use a separate /home partition

So there could be a problem with the various old systems i've tried to
install over or it could be because i just haven't upgraded U16.04.1
yet.

I hope something in here helps! Sorry it's so verbose and uninformative!
Regards from
Tom :)
