I'm having this issue as frequently as before even after OTA-12, but I couldn't find steps to reproduce the issue reliably. What happens is usually after 1-2 days after a reboot, this issue appears out of nowhere. Also deleting ubuntu-phablet-stream-volumes.tdb file is redundant, a reboot only makes things normal again.
