Public bug reported:

New Ubuntu Flash drive 1604 LTS from Ubuntu.

Installation crashes after drive selection for dual OS install.  It also
crashes for full reformat install with same errors.

My system is brand new and tested on multiple levels, all hardware is
just fine.
