Can you paste /etc/fstab from /dev/sda2 as well as the output of `ls -la
/dev/disk/by-uuid/` on the live CD? If you're willing to run through
the install again, putting set -x on the second line of
/usr/lib/ubiquity/migration-assistant/ma-ask and pasting /var/log/syslog
from the live CD after it crashes would probably help.
