1) No, it definitely did not occur in 14.04. I believe it's a regression. 2) James Ferguson noted here (https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1590985/comments/7) that he gets the bug after resuming from suspend. I will update later with more specifics, but I consistently get the bug when running Ubuntu for some time (an hour, couple of hours?), perhaps doing a suspend. I get the same behaviour like James that it's still connected, but network-manager doesn't display the active connection nor other connections. 3) Also will update later.
