Given that the last Debian release was in 2014 and the last upstream release was in 2010, I'm not sure it's fair to ask you to submit this report to either upstream or Debian (although I did find https://bugs.debian.org/cgi- bin/bugreport.cgi?bug=656105 for Debian, submitted by you, so thank you for that!).
Unfortunately, these do seem like upstream issues. It will take someone time to determine the answer to 1), at least.
