You have lots of stuff like this in your syslog:

Aug 22 09:23:47 ubuntu kernel: [ 66.181716] hdb: media error (bad sector):
status=0x51 { DriveReady SeekComplete Error }
Aug 22 09:23:47 ubuntu kernel: [ 66.181735] hdb: media error (bad sector):
error=0x30 { LastFailedSense=0x03 }
Aug 22 09:23:47 ubuntu kernel: [ 66.181746] ide: failed opcode was: unknown
Aug 22 09:23:47 ubuntu kernel: [   66.181756] end_request: I/O error, dev hdb,
sector 1427224
Aug 22 09:23:47 ubuntu kernel: [ 66.181768] Buffer I/O error on device hdb,
logical block 356806

Along with the consequent installer crash, this indicates problems
reading from the CD. I suggest burning the CD at a lower speed (if
applicable) and cleaning the CD drive lens with the sort of cleaning kit
you can get at electronics shops; then perform a CD integrity check
before starting the install again.
