As you noted yourself, this was due to a bad CD burn, as this error
indicates:
File "/usr/share/ubiquity/install.py", line 1107, in remove_extras
for line in open("/cdrom/casper/filesystem.manifest"):
IOError: [Errno 5] Input/output error
