Public bug reported:

Using the command
pdftk charSheet.pdf burst
causes form fields to become fragile in the output files, meaning that any 
attempt to add more text to a field causes the field to become invisible.  The 
data seems to still be there; it is just not displayed after clicking outside 
the field.

Example:
Original pdf:  charSheet.pdf   (I filled in a field, CHARACTER NAME on p. 1, in 
evince.)
ran command:  pdftk charSheet.pdf burst
output pdf:   pg_0001.pdf       This file still shows the filled-in field 
CHARACTER NAME.  So, in evince, if I try to add text to the same field, and 
then click away from the field, it's invisible.  Also, adding text in other 
fields behaves the same way.  If I save a new pg_001_edited.pdf, and reopen, 
the field is invisible, but the filled-in data is there.

I'm using:
Ubuntu 16.04 (64 bit)
pdftk 2.02
GNOME Document Viewer 3.18.2
