I followed the instructions given in this page: https://www.ubuntu.com/download/desktop/create-a-usb-stick-on-macos I'd downloaded 16.04.1 first. It didn't work. Downloaded 16.0.4.2 and repeated the same steps. I still saw the installer crash in both case
