thank you for your bug report and your help to make Ubuntu better.

I wanted to compare exact HW specs as this seems to be very HW specific,
so to make sure your issue is on UCS-S3260-DHBA IT-HBA as in [1]?

My Cisco products knowledge could be too bad, but the CSC-C3X60-HBA
seems to be a Node to be plugged into a UCS-S3260 as in [2], that sounds
wrong (and I assume my search is). If you'd have the right product for
your case you referred working with 16.04 that would be kind.

Do you have a way to get into the installation prior to (the failing) reboot?
It might be interesting to get some logs from there.
Feel free to add what you consider important, but actually on the install 
nothing is confidential so you could just tar up all in /var/log of the install 
environment.

If there is anything reasonable on the install console feel free to add
that as well.

And since we are talking about multipath issues with mp while installing
maybe a "multipath -v3 -ll" from the good and the bad system to compare
the disk/controllers as they were detected could help a lot.
