I was able to solve the problem by booting with an old kernel, editing /etc/initramfs-tools/initramfs.conf and replacing MODULES=most with MODULES=dep and then running sudo update-initramfs -u. I think the problem was that initramfs was including a module xhci-plat-hcd which is buggy and not really required. I was seeing error messages of the type ... xhci_hcd ... HC died; cleaning up and then couldn't enter my passphrase. My computer is a MacBookPro 12,1.
