I tried starting it as firefox --safe-mode mode instead. The problem still exists, although it looks a little different, presumably because the Menu Bar isn't enabled. It starts up without any space between the title bar and the tabs. Right clicking above the tab gives the pop-up menu but with Menu Bar, Bookmarks Toolbar and Customize greyed out. Selecting Menu Bar produces some space where the Menu Bar would go, but no menu bar. The option in the right-click menu then has a tick beside it.
