Public bug reported:

A colleague of mine was trying to install Ubuntu 16.04, but the
installation wizard would get stuck. mkfs.ext2 was stuck reading on
stdin. We ran the same command in a shell, and found that it printed
something like this:

Found a dos partition table in <partition>
Proceed anyway? (y,n)

and was waiting for the input of the user. He was unlucky enough that
the installer would reuse a similar partition layout as the system he
installed before in non-UEFI mode. I didn't find a "non-interactive
mode" option for mkfs.ext2, but if it exists the installer should use
it. Otherwise, maybe pipe the "yes" command in it.

If that matters, he selected "Use entire disk" with full disk encryption
enabled.
