OK, been having this problem for a while now and have finally narrowed
it down so that I can repeat it. Using Kubuntu 7.10. Steps for me to
reproduce are:

Double-click a movie file to open it in Kaffeine
Drag another movie file from the dolphin window; window placement needs to be 
in such a way that the tooltip from hovering over the file overlaps the 
kaffeine window
Kaffeine grabs focus but tooltip remains on top of kaffeine window (I have 
focus follows mouse if it makes any difference)
If you hover the cursor over the remaining tooltip, dolphin will always crash 
with the following backtrace:

Similarly, if I fullscreen kaffeine by double-clicking, the tooltip
remains on the screen overaying the video; one again placing the cursor
over it will result in dolphin crashing in the background.

I imagine this particular set of circumstances might be unlikely (not
been able to accidentally reproduce it with other drag'n'drop actions),
but it looks like an unhandled focus exception somewhere...
