Public bug reported:

Ubuntu installer continuously crashes when attempting to install ubuntu
16.04 (or later) on Lenovo D30 Thinkstation. Disk previously zero'd out
using the following command sudo dd if=/dev/zero of=/dev/sda bs=4096
status=progess. Once the drive and partition table were removed I
reformatted disk as EXT4 then attempted to install from a live USB.

Answers to the "We also need" section, below:
1. Ubuntu 16.04 LTS.
2. The output of apt-cache policy pkgname was "N: Unable to locate package 
pkgname."
3. I expected Ubuntu to install.
4. It did not install correctly.
