"attempt to read or write outside of disk" is usually caused by creating
partitions/file-systems that extend beyond the limits of the PC's BIOS
disk access functionality. The limit depends on the particular BIOS but
is usually around 137GB (128GiB).

If any file-system required for starting Linux extends beyond this point
on the disk it may fail when GRUB is relying on the PC's own BIOS to
read sectors from the device.
