Tested, it  is working fine in both xenial and zesty.

3) Recreation steps:
i) Prep a guest
ii) Remove in the os section
<boot dev='hd'/>
iii) Add this on your former primary disk:
<boot order='1'/>
iv) And then add the xml content generated with:

h=$(printf "\x$(printf %x $((98+$i)))")
echo "<disk type='file' device='disk'><driver name='qemu' type='qcow2'/><source

qemu-img create -f qcow2

4)will be attaching guest xml and ouput of lsblk command output from the
guest.
