I am seeing this on 16.04 on a fresh install. I suspect that there's an interaction with the graphics drivers which is why the meagre attempts above to reproduce the problem have failed. You need to be running on a susceptible computer, in my case it's an Acer Aspire 5332. It would probably help if other posters returned to add in their computer's details.
