Another forum gave me an idea.  I dual boot from Vista, but if I
shutdown, then unplug the power cord, boot to Kubuntu, then plug the
cord back in, the sound **seems** to work.  I've only reproduced this
twice so I'm not ready to call it a "workaround" but it does seem to
have merit.  The reasoning was that Kubuntu couldn't reset the sound
hardware after a warm boot from Windows.
