Thanks. I have seen other audio bugs with the same problem -- the state
of the hardware changes on warm boot. But hopefully there is a software
solution...

Can you please look at System Settings > Sound and verify that 'Allow
louder than 100%' is OFF? If it is ON then that would explain the
problem.

Please also try toggling that tick box and playing with the volume
slider to see if you can make the distortion go away.
