Newer versions of nvidia-modprobe create a device at /dev/nvidia-uvm-
tools. This was added in nvidia-modprobe 364.12

Missing this device causes issues with nvidia-docker, as seen at

and others.

Attached is a udev rule to properly create this device, which would also
allow the create-uvm-dev-node script to be removed.
