A whole year later I'm still affected by this bug with Kubuntu 18.04.
No volume notification shows up in kde but audio playback still skips about 
once or twice in in a couple of minutes like headphones just got plugged in and 
then immediately taken out.
No headphones were plugged in at any time by the way.

Workaround in comment #20 still works.

My sound card:
(lspci) Audio device: Intel Corporation 7 Series/C216 Chipset Family High 
Definition Audio Controller (rev 04)
(aplay -L) HDA Intel PCH, ALC898 Analog

I never used Windows on this machine so I can't rule out the possibility
that this is actually a hardware problem.
