Some more insight; with a Xenial/Ocata deployed cloud with the following
software versions:

ceph/librbd 10.2.9
libvirt:  2.5.0-3ubuntu5.6
qemu: 1:2.8+dfsg-3ubuntu2.9

I can hard kill a qemu instance, and have it boot back from a ceph
volume OK.

I suspect this is something in the interaction between qemu and librbd
in later software versions.
