Your SSD has failed and probably needs replaced. In addition, your
installation medium is corrupt; you will need to recreate it and may
want to verify it using the check disc option at the boot menu.

install from usb have bug on ubuntu studio 16,18 attached bug report.
Checked my requiement and found another ssd bug but have same problem
on scsi and ide drive as well. i am not sure hdd occupy those
problems. if possible any advice will be appreciated. thanks
