Public bug reported:

new install of 18.04.  This is an old issue dating back to at least
14.04.  There's another ticket concerning this somewhere (I know, I
reported it)  I'm unable to find it now (sorry).

The crash happens when btrfs is on /boot.

System is a Macbook Pro 9,1, can be duplicated on all my systems using
(U)EFI.
