It seems that evince has supplied a wrong name for the paper size. See
these two lines in the error_log:

The client has sent the job with a "PageSize=Custom.Letter.SM" attribute
and it should have used "Letter.SM". In the second line you see that in
the Ghostscript command line zero got inserted for the width and the
height of the page, due to the broken page size name.
This is a bug of the client. As it occurs with evince it is most
probably the print dialog of GTK. Adding GTK task ...
