I had already installed Ubuntu 18.04 LTS on /dev/sdb once, but the grub
install failed, so I was reinstalling it, and so I chose "Erase disk and
install 18.04 LTS over Ubuntu 18.04" or whatever it said on the screen.
The implication was that the installer would auto-partition the 500GB
disk and install Ubuntu into that newly partitioned disk. So I did not
manually partition the disk. I intentionally let the installer overwrite
the previous partitions on the disk.

During the installation, the installer failed to install the bootloader,
so I tried various options from the dialog that was displayed, but they
failed. So I opened a Terminal window and tried manually installing the
bootloader, and that's when I got the "/cow" error. And perhaps that's
when the installer crashed and sent this report.
