It 'looks like' Ubuntu 18.04 was used, but the kernel and package versions do
not fit and/or are on a different level:

libvirt0:ppc64el 4.0.0-1ubuntu8.6
respectively:
Kernel: 4.15.0-1016.18-fix1-ibm-gt
qemu: 1:2.11+dfsg-1ubuntu7.8-1ibm3
libvirt-bin : 4.0.0-1ubuntu8.6
Official levels on bionic are today:
linux-generic | 4.15.0.46.48 | bionic-updates | ppc64el
qemu-kvm | 1:2.11+dfsg-1ubuntu7.10 | bionic-updates | ppc64el
libvirt-bin | 4.0.0-1ubuntu8.7 | bionic-updates | ppc64el
Is in your environment only the above kernel patch required for fixing
this, or are also patches in qemu (and maybe even in libvirt) needed?
Since you are using at least an own and non-standard kernel and qemu
package.
I'm wondering why the versions in use are different from the ones in the Ubuntu
archive ?
I think a retest with the latest standard Ubuntu packages needs to be done to
ensure that the situation is the same (which I assume, but anyway).
