Have you checked the health of you drive?
(https://help.ubuntu.com/community/Smartmontools)

Have you booted 'live' media (eg. Ubuntu install media) and checked the
file system for logical errors? (ie. `fsck`) assuming you're drive(s),
PSU & hardware is healthy

etc.  (ie. normal support options)

I'm going to mark this bug incomplete, as it appears as if you haven't
followed the normal support procedures first. If however you believe I'm
in error, please leave a comment with details and change the status back
to "New".
