DMAR table of Haswell macbookpro11,1 [8086:0a2e]

I have been able to reproduce this bug on a Haswell system, specifically
a 2013 Retina MacBookPro (macbookpro11,1) with an integrated Intel
graphics card, with a single HDMI out.

00:02.0 VGA compatible controller [0300]: Intel Corporation Haswell-ULT
Integrated Graphics Controller [8086:0a2e] (rev 09)
00:03.0 Audio device [0403]: Intel Corporation Haswell-ULT HD Audio Controller
[8086:0a0c] (rev 09)

(I'm attaching the full lspci -nn)

I can reliably reproduce the symptoms of lagging audio and eventual
complete failure of HDMI audio. I can work-around this issue by
disabling IOMMU either completely or just for the integrated graphics.

I haven't test the kernel patch, but I can do it if you believe it is
still relevant to try and fix this upstream.

I hope I can help with this, thank you very much for your work!

I'm attaching the DMAR table of my system, as well as my boot/kernel log
for IOMMU-off and IOMMU-on
