Steps to reproduce:
===================
 1. Open a docx containing forms such as the attached IPR declaration form.

 2. Fill out some of the form fields.

 3. Save the file and reopen it with LibreOffice writer.

 4. Form fields that have been filled out are no longer visible.


Fix:
====
- Updating LibreOffice to 6.4.4 (either from the official site or from 
ppa:libreoffice/ppa fixes this problem.


Further Notes:
==============
- The form data can actually be recovered by opening the file with AbiWord (to 
my best knowledge, neighter Microsoft Word nor the fixed version of LibreOffice 
are able to recover the data).
