Hi Alex,
Thanks for filing this bug report! I just launched a trusty instance,
and both of the entries in menu.lst look like they should not trigger
this bug ("root\t(hd0)"). I then successfully upgraded the instance to
xenial without seeing this issue, and I still see "root\t(hd0)" in all
the menu.lst entries.
Can you give us some more details about how we might be able to
reproduce this? In particular, instance type and original AMI would be
very helpful. Once you've done so, please move this bug back to New.
