ok, it broke after an ubuntu update. After about 3 weeks of looking for the issue and installing and uninstalling akvcam 2 times. Today, I managed to resolve it by installing akvcam one more time. I suspect there was recently a few ubuntu updates which were colliding/overwriting some code used by akvcam code.
