Thank you very much for the sample file. I have printed it into a PDF file from evince to be able to do further tests manually on the command line.
This way I have done some tests and found out that adding "-dDEVICEWIDTHPOINTS=XXX -dDEVICEHEIGHTPOINTS=YYY" with any non-zero values for width and height works (giving the correct page count), small values make it even faster.
