You can also find help with your problem in the support forum of your local Ubuntu community http://loco.ubuntu.com/ or asking at https://askubuntu.com or https://ubuntuforums.org, or for more support options please look at https://discourse.ubuntu.com/t/community-support/709 I'd upgrade your system; as the 5.11 kernel is no longer supported by Ubuntu (it was a 21.04 kernel which is now EOL; with 20.04 systems having upgraded to 5.13 some weeks ago now). Your `ubiquity` crash I suspect relates to how your live-with- persistence was created (ie. bug maybe belongs with whatever program you created it), and if your upgrades are not being applied (ie. why you're still using an EOL 5.11 kernel) that is more symptoms of issues with your persistence-media creation. I've marked this bug as 'Incomplete' as details show an non-updated system with possible issues in creation of the live media with persistence.
