By the way, there's a deeply buried option for window placement:
System Settings -> Window Management -> Window Behavior -> Advanced -> Window 
Placement

Regardless of that option, some dialogs, e.g. the master password
dialogs of KWallet and Thunderbird, are always placed top left. The one
of Firefox is centered, though. After a Plasma update, the KWallet
dialog moved even further to the left. If the mouse had not yet been
moved after login, windows used to open on my primary screen. Now they
open on my secondary screen (which is the left-most).

Regarding window rules: as long as applications don't set distinct
window classes for different kinds of windows, they won't be really
helpful.
