Public bug reported:

When performing a do-release-upgrade from 16.04 to 18.04 I encountered
many errors pertaining to duplicate keys and columns. I finally aborted
the futile process when the script complained about not being able to
drop a column because it did not exist. It did not exist because I had
to drop the column when it was complaining about it being a duplicate.
