Thanks for the bug report.

It sounds like extensions have been automatically disabled after waking
from sleep. This happens if one or more of them experience errors (bugs)
when being re-enabled during wake up.

Please start by deleting all local extensions:

  cd ~/.local/share/gnome-shell/
  rm -rf extensions

and then log in again. I expect that will fix the issue and you can then
reintroduce your custom extensions one by one.

Please also make sure you never have dash-to-dock and ubuntu-dock enabled at 
the same time as they will conflict with each other (bug 2003168). There's a 
chance that explains this whole bug.
