Not sure why this happens, but I would probably try removing
/lib/terminfo/w/wsvt25m (because ultimately you want something in
/usr/), and then run /usr/lib/usrmerge/convert-usrmerge as the error
output suggests:

Both /lib/terminfo/w/wsvt25m and /usr/lib/terminfo/w/wsvt25m exist.
You can try correcting the errors reported and running again
/usr/lib/usrmerge/convert-usrmerge until it will complete without errors.
